This has the most violence of the three current books in this series. There are several instances of violence throughout the book, some of which are quite graphic and include hurting children. There are violent deaths of both people and creatures as a supernatural war is fought.
There is mention of adult characters smoking. Several mentions of adult characters drinking and one time where the protagonist (who is under 21) has a drink, which is from another realm, but tastes like alcohol, hence the 3 rating. (note - the protagonist has drunk alcohol in the previous books which added to the assumption made).

Other: This is on the upper end of YA novels with the protagonist being 18. In this installment there is more violence than in previous installments but nothing description wise that is anymore graphic. It does deal with far more in the way of Supernatural elements just because of the events that have happened prior.
